How to Make a Consultation

How to make a appointmentAs specialists we would first require a referral from your normal dentist, GP or hospital.

Your dentist or GP can complete the online referral form and then one of our reception team will contact you to arrange a convenient time for your consultation.

If you have been told by your dentist or GP that you need to see one of our specialists please contact our reception team on 08 9328 3006 and they will be ready to help you arrange a convenient time.

What you will need to bring with you to your consultation:

  • Your Medicare card - this is important for claiming purposes
  • Your Health fund card - this again is important for your claiming purposes
  • A list of medication you are currently taking - this is very important for treatment planning and if appropriate you may be asked to see one of consulting Anaesthetists.
  • If you have been given your x-ray please make sure that you bring this with you (this is very important as the surgeon may not be able to see you without this)
  • Your referral letter if the doctor or dentist has not referred you via our website.
